Our Pledge Regarding Protected Health Information
We understand that protected health information about you and your health is personal. We are committed to protecting your protected health information. In order to provide you with quality care and to comply with legal requirements, we create a record of the care and services you receive from accessAbilities. This notice applies to all of the records of your care maintained by accessAbilities. Your other health care providers, such as your personal doctor, may have different policies or notices regarding the use and disclosure of your protected health information created and maintained in the doctor’s own office or clinic.
The Provider is required by law to:
- Safeguard your protected health information;
- Give you this notice of our legal duties and privacy practices with respect to your protected health information;
- Follow the terms of this notice as currently in effect;
- And, notify you of any changes to this notice.
